The following documentation will be required when you submit the application:

Copy of birth certificate for each family member (only for members under the age of 62)

Copy of social security card for each family member
- Name and address of your employer and 4 to 6 paystubs for each member that is employed (if applicable)

Copy of social security or TANF or food stamps award letter on each adult member (if applicable)

Name and address including your account numbers from your bank. On each adult member
